A new kind of hope:

The age-old concept of neighbors helping neighbors is alive and well, thanks to Cedar Community’s newest division. This off-site expansion of Cedar Community brings our strong tradition of compassionate, trusted care right to the doorstep of people of all ages in Washington, Sheboygan, Ozaukee, Waukesha, Dodge and Fond du Lac counties.

Our five service lines include:
  • Home Health
    (comprehensive medical services)
  • Home Hospice
    (for those facing end of life)
  • Supportive Care
    (non-medical chores, companion services, errands, etc.)
  • Palliative Care
    (for those under treatment for life-limiting illness)
  • Cedar Care Pathfinders
    (professional care manager)

Our staff includes a complete spectrum of licensed, experienced professionals:

  • Registered and licensed practical nurses
  • Physical, occupational and speech therapists
  • Social workers
  • Home health aides, companions, escorts
  • Chaplains and volunteers
Payment arrangements:

Cedar Community Home Health and Hospice is Medicare- and Medicaid – certified and licensed by the state of Wisconsin. Payment sources may include:

  • Medicare
  • Medicaid
  • Private insurance
  • Private pay
