A Cedar Care Pathfinder geriatric care manager is your sage resource for identifying and obtaining the most appropriate care and support. This experienced social worker is licensed and trained to help seniors and their families navigate the many resources available today to meet needs and maintain independence. Your Pathfinder will accompany you to medical appointments, summarize information, advocate on your behalf, create a medical care or supportive care plan, and help research and arrange care providers or moves to appropriate housing.
