Cedar Crossings Subacute Care
Your bridge to recovery - now expanded to two locations
When you're ready to leave the hospital, but not quite ready to return home, Cedar Crossings is there. Located within the Cedar Lake Health and Rehabilitation Center off Highway Z in West Bend, and now in its second location in the Pleasant Valley Medical Building, 3200 Pleasant Valley in West Bend, Cedar Crossings Subacute Care caters to the needs of adults age 21 and older recovering from illness, surgery or injury. Length of stay ranges from five to 40 days, with discharge to the patient's original home setting or another appropriate level of care. One of the area's largest, Cedar Crossings Subacute admits short-term care patients requiring medical monitoring, nursing care or rehabilitation without other significant medical complications.
Subacute services:
Those considering surgery are encouraged to make plans for subacute pre-admission. Free facility tours are available, or the admissions coordinator will meet with you and your loved ones at home or in the hospital. Medicare, managed care or private insurance generally covers our comprehensive and cost-effective programs.
