What services does Cedar Community’s Home Health program include?

Cedar Community partners with a not-for-profit home health, hospice, and palliative care organization serving Wisconsin. Their home health program provides nursing care, medication management, physical/occupational/speech therapy, and assistance with daily activities delivered in your home. Care is coordinated by a registered nurse in collaboration with your physician.

How do hospice services at Cedar Community support patients and families?

Hospice focuses on comfort and quality of life for individuals with life‑limiting illnesses. Our partner’s hospice team–nurses, social workers, chaplains, and bereavement coordinators– provide pain management, emotional and spiritual support, and guidance for family members at home or on campus.

Who provides care and how often do visits occur?

A multidisciplinary team of nurses, therapists, social workers, aides and chaplains provides care. Visit frequency depends on the patient’s plan of care and may range from several times per week to daily support.

Where can care be delivered—at home or on campus?

Home health and hospice services are available both in residents’ homes and at Cedar Community campuses. The hospice team can also coordinate transfers to Cedar Community’s skilled nursing facility if needed.

How do I know if I qualify for home health or hospice?

Eligibility depends on medical diagnosis, functional limitations, and physician orders. Care coordinators can speak with your doctor and insurance provider to determine eligibility.
